根拠

　センチネルリンパ節の同定法には色素法単独、RI法単独、色素＋RI併用法があり、いずれの方法においてもその有用性が報告されているものの、乳癌診療ガイドライン、外科療法2005年版では併用療法を推奨している。その根拠としては、(1)文献検索上方法間の比較検討において結果は一定しておらず、色素法単独と併用法の結果は同じとする報告や、併用法が色素法単独よりも同定率、偽陰性率において優れているとする報告もあること、(2)しかしその中でも大規模サンプルに対する検討では併用法のほうが優れているとしている傾向があること、をあげている。また、いずれの方法も外科医の経験数が同定率に影響することも指摘されており、さらにセンチネルリンパ節の所見が術式を大きく変える重要な情報であることを考慮すると、併用法において正確な同定を行うことが必要であると考えられる。

　以上より、センチネルリンパ節の同定には色素とアイソトープの併用法を用いることをQIとした。
