根拠

 ICG停滞率を肝機能の評価に使用することは広く行われている。科学的根拠に基づく肝癌診療ガイドラインにおいても、ICGによる術前肝機能評価因子としての有用性が指摘されている。その根拠として、術後合併症の有無ではICG 15分停滞率に有意差はみられないものの術後死亡との関連ではT-Bil、ALT、γ-GTPとともにICG 15分停滞率が有意な予測因子であり、特にICG 15分停滞率が最も良い予測因子であったとの報告を引用している。ICG停滞率は腹水などの門脈圧亢進の所見がなく、ビリルビンが正常な症例における肝切除の範囲の大きさを決定する上でも有用であり、肝切除の計画にあたって必須の検査といえる。
 以上、ICG 15分停滞率は術後死亡の優れた予測因子であり、手術適応などを含む治療方針の決定に有用であることから、肝切除術を受ける肝細胞癌患者は、ICG 15分停滞率が測定されるべきである。
