分母:      肝障害度A、最大腫瘍径3cm以下、個数2~3個の再発肝細胞癌患者数

根拠

 特に再発性の肝細胞癌患者を対象に、肝切除術、局所療法(RFA、MCT、PEI)、TA(C)E、対症療法のうち最適な治療法を検討したRCTは存在しない。3年生存率は再肝切除術で59~80%、RFA/MCTで43~78%、PEIで22~84%、TA(C)E で24~ 48%である。手術可能な再発肝細胞癌(個数3個以下であり、Child-Pugh分類、ICG 15分値で判断。基準の詳細は不明)に対して肝切除術とTA(C)Eを比較した小規模試験では、3年生存率は肝切除術とTA(C)Eで有意差を認めなかった。また肝切除術、TA(C)E、PEI、化学療法、保存療法を比較した小規模試験では有意差はないものの、肝切除術が最も生存率が良好であり、次にTA(C)Eであった。最近の研究では個数3個以下の再発肝細胞癌患者106例に対する肝切除術、PEI/RFA、TA(C)Eを比較し、各生存曲線に有意差を認めなかった。単発性の再発肝細胞癌患者については、根治を目指した治療をすべきである。TA(C)Eについては、一般に根治性が肝切除術、局所療法よりも劣ると考えられており、状態のよい再発例を対象とした明らかな違いは、研究で示されていないものの、条件のよい再発例に対して、許容可能とするかどうかは検討の余地がある。
 以上、肝障害度Aの腫瘍径3cm以下、個数2~3個以下の再発肝細胞癌患者は、根治療法のよい適応であることから、肝切除術または局所療法(PEI、MCTまたはRFA)またはTA(C)E が行われるか、行われない場合には理由が診療録に記載されるべきである。
