■変更理由
プラチナ製剤と組み合わせる薬剤に関しては、現在は様々あるので限定するべきではないし、また、二剤とも限らないので、「多剤併用療法」とした。ただし、EGFR変異のある患者に対してはゲフィチニブが1stラインの化学療法でもかまわない。これは「行われない根拠」として扱われる。PSの記載がない場合にはPS0~1と判断して分母に含める考え方もあるが、PSは重要な項目であることから、明確にPS0~1のみの患者を対象にする方針とした

根拠

 メタアナリシスにおいて、進行非小細胞癌では化学療法により有意に生存期間が延長することが示されている。第3世代抗癌剤(塩酸イリノテカン、ビノレルビン、ゲムシタビン、パクリタキセル、ドセタキセル)とシスプラチンとの2剤併用療法とシスプラチン単剤との比較試験では、併用療法がシスプラチン単剤に比べ、有意に生存期間が延長することが認められている。第2世代抗癌剤(ビンデシン、イフォマイド、マイトマイシン、エトポシドなど)とプラチナ製剤併用療法、第3世代抗癌剤とプラチナ製剤併用療法との比較試験では、第3世代抗癌剤とプラチナ製剤併用療法が生存率において有意に優れるとの報告が多く、メタアナリシスにおいては第3世代抗癌剤とプラチナ製剤併用療法がわずかながら勝っていることが示されている。第3世代抗癌剤とプラチナ製剤併用療法同士の比較試験では、いずれのレジメンにおいても効果に差がないことが示されている。また、EGFR変異のある場合ではゲフィチニブが1stラインの化学療法にもなりえるが、その旨が診療録に記載されるべきである。

 以上より、化学療法施行可能な患者において、プラチナ製剤を含む多剤併用療法は、単剤療法よりも予後の改善を認めていることから、75歳未満、PS 0~1でStage III~IVの非小細胞癌と診断された患者に化学療法を行う場合、プラチナ製剤を含む多剤併用療法(塩酸イリノテカン、ビノレルビン、ゲムシタビン、パクリタキセル、ドセタキセル)が行われるか、行われない場合には根拠が診療録に記載されるべきである。
