根拠

　StageⅠ非小細胞癌の放射線治療に関する系統的レビューでは３年、５年生存率は各34＋９％（平均値＋1S.E.）、21＋８％、原病生存率は各、34＋10％、25＋９％であった。わが国において1980～1989年に55～75Gyの通常照射で治療されたStageⅠ非小細胞癌の５年生存率は22％であった。EBMに基づく肺癌診療ガイドラインの系統的レビューでは、検索した９件のガイドラインのうちStageⅠに対して６件、IIに対して５件のガイドラインで非手術症例に対して放射線治療の適応があると記載しており、何らかの理由で手術ができないStageⅠ／II非小細胞癌に対しては経過観察よりも根治的放射線単独治療が推奨されると考えられる。

　以上より、非手術症例においても放射線治療により良好な生存率が期待できることから、臨床StageⅠの非小細胞癌と診断された患者で手術が行われないならば、根治的放射線単独治療が行われるか、行われない理由が診療録に記載されるべきである。
