根拠

　シクロホスファミド単剤とシクロホスファミドを含む多剤併用化学療法の比較では多剤併用化学療法が有意に生存期間の延長を認めている。肺癌診療ガイドライン2005年版ではシスプラチンとエトポシドの併用療法およびシスプラチンと塩酸イリノテカンの２剤併用療法を標準的治療として行うよう勧めている。また、PS不良例（PS 2、3）に対して経口エトポシド単剤と多剤併用化学療法との比較試験が行われたが、経口エトポシド群は多剤併用化学療法群と比較し、生存が不良で、毒性も強いことが示された。これらの結果より、小細胞癌に対する化学療法は多剤併用療法が推奨され、PS不良例に対しても可能であれば併用化学療法を行うべきである。

　以上より、小細胞癌に対して多剤併用療法は単剤化学療法に比べて生存期間の延長が認められることから、小細胞癌と診断されたPS0～3の患者には多剤併用化学療法（化学放射線療法の場合も含む）を行うか、行わない場合はその理由が診療録に記載されるべきである。
