根拠

 集中治療室患者での院内尿路感染症のリスク因子を評価した前向きコホート研究では、尿道カテーテル留置期間が独立した感染のリスク因子であった。1日当たり3~10%の細菌尿の発生があることから、4日以内のカテーテル抜去が推奨されており、またカテーテルを5日以上留置された患者のうち50%が無症候性菌血症、カンジダ尿症を発症し、抗生剤抵抗性細菌の温床となっていたとの報告もある。

 結腸癌の手術においては、手術操作による術後排尿機能障害の可能性はほとんどなく、影響を及ぼす他の合併症がない限り、離床を促す意味でも早期のカテーテル抜去が推奨される。カテーテル抜去については、担当医が尿路感染症の予防を意識していることが重要であり、漫然と長期留置することと区別するために、長期留置が必要な場合には、その理由を診療録に記載することも重要であると考える。

 以上より、長期の尿道カテーテル留置は、離床の妨げになるとともに尿路感染症の発生を助長することから、手術を受けた結腸癌患者では、術中に尿道カテーテルが留置された場合、術後4日目までにカテーテルが抜去されるか、抜去されない場合はその理由が診療録に記載されるべきである。
